Medical Plan

  • Full Coverage Health Plan - Provides comprehensive health coverage (medical, pharmacy, dental and vision) for employees and their families
  • Supplemental Health Plan - Supplemental medical and pharmacy coverage benefits for employees and their families who have other health coverage (for example, through a spouse's employer, Indian Health Services, Military, other group plan, etc.)

    Life and Accidental Death and Dismemberment (AD&D) Insurance

  • Basic Life - Two and one-half (2.5) times employees' annual base salary ($350,000 maximum)
  • AD&D - Up to two and one-half (2.5) times employees' annual base salary for loss of limb or sight as the result of an accidental injury
  • Supplemental Life - Employees are also eligible to purchase additional coverage for themselves, spouse, and child(ren). Costs are based on employee age.
  • Disability - Loss of Income Protection

  • Short Term Disability - Provides 67% of pre-disability weekly earnings, up to a maximum of $2500/week
  • Long Term Disability - Provides 60% of salary, up to a maximum of $10,000/month

    Paid Time-Off (PTO)

    A generous Paid Time Off (PTO) program is provided for use as personal time off, including vacation, or to cover the occasional short-term illness:
  • 0-3 yrs -- .088 per hour worked -- 184 hours
  • 3-6 yrs -- .108 per hour worked -- 224 hours
  • 6+ yrs -- .146 per hour worked -- 304 hours
  • PTO starts accruing the first day of employment and is available for use after 90 days (maximum accruals apply).

    Retirement Plans

  • Pension Plan - 401(a) - 6% of eligible compensation based on employment on December 31 annually
  • Voluntary 403(b) Savings Match - If you elect to participate in a voluntary 403 (b) savings program, YKHC will match your contribution up to 2% of gross salary
  • 403(b) Retirement Plan: All current employees are eligible to participate in the plan upon employment
