Diabetes Prevention & Control
Diabetes Prevention and Control Program
YKHC's Diabetes Prevention and Control Program serves 48 villages in southwest Alaska's Yukon-Kuskokwim Delta - an area the size of the state of Oregon. Approximately 90% of the area population is Alaska Native, including Yup'ik, Cup'ik, and Athabaskan.
Our program offers a variety of outreach and clinical services to address the changing needs of the people. Our staff includes: Clinical Diabetes Educators, Outreach Coordinators, and Program Coordinator. Meet the diabetes team.
Outreach coordinators:
- provide diabetes education presentations at schools, Head Start programs, pre-maternal home and in the community organizations, PATC, tribal gatherings and upon request;
- conduct blood glucose screenings at health clinics, health fairs, community events and upon request;
- work with communities to encourage physical activity like the exercise program at the Senior Center in Bethel, or Diabetes Prevention Ski Health Fair in Oscarville;
- provide schools and communities with physical activity awards or garden awards.
- The deadline to apply for the 2008 School Physical Activity Award application was October 20, 2008.
The deadline to apply for the 2009 garden award was February 27, 2009.
The deadline to apply for the 2009 community physical activity award was March 13, 2009.
Clinical diabetes educators:
- provide diabetes self-management education for clients and their families;
- provide both inpatient and outpatient services;
- identify and educate individuals at high risk for diabetes;
- function as a resource for the health care team providing patient care;
- offer pre-pregnancy counseling and management;
- serve as patient advocate in delivery of diabetes care;
- work towards gestational diabetes management;
- provide medical nutrition therapy;
- plan and participate in multi-disciplinary "one-stop" sub-regional clinics in which patients receive a variety of services.
As a team we:
- coordinate the monthly diabetes support group, every second Tuesday from 6 PM to 8 PM at YKHC's Board Room, on the third floor of the CHSB - across from the hospital;
- host the monthly diabetes radio show, every second Wednesday from 10 AM to 11 PM on KYUK;
- facilitate special community events, like the Senior Health & Fitness Day and the Family Health & Fitness Day
